Creating relief map texture in Photoshop

I've been working through Jeremy Elford's tutorial for making a relief map in PS, but the tut is for an older version. The nvidia plug in is out of date and I've read you can make a normal map in PS, but when I do this is makes a complete 3d model. I'm struggling to understand the steps from taking the image from flat with my colours, to makng it raised, like the picture.
