The islands and such is just a thing of practice and getting used to placing them.
I try various locations, sometimes erasing and replacing them.
They don't always look quite right so i fiddle until i am satisfied with them.
I look at a lot of satellite images too.

It is a slow process and one that you have to work into.
It's a balancing act of detail and some blank space.
