So this is another thing we all have to be careful about?

Most clients are probably perfectly ok, but if you just happen to engage with the wrong one... hmmm.

I think if I was ever to start doing commission work I would have to do a Google image match before I accepted - just to be sure

I was literally given an outline to work from which wouldn't do much good in a reverse image search, and the client referred to the world as Eridanus, not Toril:

I don't personally believe that they did anything wrong, as it was made clear to me that this was commission for personal use, for an in house campaign. So long as it is for personal use I don't see the issue, but IANAL.

I can see what you're saying, but even so - it would have been better if they had told you it was a copy of Toril, instead of leaving you in the dark.
